Job summary

LHC Group, part of the Optum family, provides home health, hospice and community-based services that help people receive quality care at home. The Senior Living Physical Therapist will perform age-appropriate assessments, provide direct patient care, supervise PTA/tech, and develop home exercise programs, while ensuring safety and documentation.

You will participate in team conferences, manage adaptive equipment, and educate patients and families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient to determine rehab potential and develops an individualized plan of care.
• Sets initial STG's and LTG's.
• Utilizes safe treatment techniques and maintains equipment logs.
• Completes discharge summaries on final visits.
• Provides patient/family instruction and participates in team and family conferences.
