Job summary

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver''s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient to determine rehab potential, identifies needs, and develops an individualized treatment plan.
• Sets initial short- and long-term goals and monitors progress.
• Uses safe treatment techniques and maintains equipment logs and safety.
• Completes discharge summary on final outpatient visit.
