Job summary

Reliant Rehabilitation is seeking an Occupational Therapist to provide therapy and related services in an Assisted Living, Independent Living, and Memory Care setting. You will screen, evaluate, and treat residents, develop plans of care, and discharge planning while adhering to state and federal rules.

You will supervise COTA's and rehab techs, partner with an interdisciplinary team, and ensure accurate documentation and compliance with OBRA and company policy.

Qualifications

• Competent clinical problem-solving and teamwork.
• Strong communication with diverse teams.
• English reading/writing proficiency required.

Responsibilities

• Screen and evaluate residents for therapy.
• Develop and manage initial/ongoing treatment plans.
• Discharge planning and continuity of care.
• Supervise assistants and rehab techs.
• Coordinate with COTA staff and interdisciplinary teams.
• Ensure compliant billing/documentation per regulations.
• Travel between communities as needed.
• Maintain patient confidentiality and privacy.
