Qualifications

• Current state LPN/LVN or RN license if required by State.
• Experience working with elderly in healthcare settings.
• Supervisory experience preferred.
• Ability to establish effective relationships with residents, family members, and staff.
• Strong documentation skills.
• Ability to represent the facility in a positive and professional manner.

Responsibilities

• Monitors the daily care of all residents.
• Performs skilled nursing tasks as defined by policy and state law.
• Documents observations, concerns, and changes in resident conditions, incidents, and/or accidents and ensures follow-up until resolved.
• Communicates resident needs and concerns to family members and physicians.
• Communicate with family members, physicians, and other outside healthcare providers.
• Records and implements physician orders.
• Ensures information on resident service plans is implemented by caregivers.
• Ensures Medication Aides maintain an orderly medication cart and records.
• Receives reports from caregivers before the end of the shift.
• Provides status reports to oncoming staff.
• Assist in shift assignments and ensure duties are completed satisfactorily.
• Monitor care staff performance for policy compliance and quality care.
• Manage shift coverage when staff call in sick.
• Schedule and other staffing duties as requested.
• Address family/visitor concerns with proper reporting.
• Manage minor facility issues and coordinate with Maintenance.
