Job summary

Curana Health seeks a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living facilities. You will provide direct patient care, manage chronic conditions, and coordinate with other healthcare providers to promote wellness and independence for residents.

You will perform comprehensive assessments, develop treatment plans, and participate in rounds and care coordination to optimize outcomes.

Responsibilities

• Provide comprehensive patient care and ongoing care management for residents.
• Perform routine rounds and monitor residents' conditions and adjust treatment plans.
• Coordinate care with facility staff, families, and other providers to ensure comprehensive care.
• Educate residents and families on disease prevention and wellness.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care and reduce readmissions.
