Job summary

Via Elegante Assisted Living in Tucson, AZ, seeks a Health & Wellness Director to lead clinical care in our memory care and assisted living community. You will oversee resident care, medication management, and care plans, partnering with families and providers to safeguard safety, dignity, and quality of life.

This hands-on leadership role requires defining care standards, mentoring a care team, ensuring regulatory compliance, and maintaining detailed records.

Qualifications

• Must have a high school diploma or GED.
• Arizona Caregiver Certification or LPN license or equivalent experience.
• Arizona Memory Care Certificate or ability to obtain within 30 days.
• Current First Aid/CPR certification at hire.
• Arizona fingerprint clearance card required.

Responsibilities

• Oversee resident care and medication management.
• Develop and review individualized Resident Service Plans with families and providers.
• Coordinate care across transitions with home health and hospice as needed.
• Lead care conferences and mentor the care team.
• Ensure documentation accuracy and regulatory compliance.
• Provide on-call support and respond to resident/family concerns.
• Maintain a positive, collaborative team culture.
