Job summary

Viva Senior Living in New York is seeking a Case Manager in Clinical Services to evaluate resident needs, orient new residents to routines, and coordinate care planning with families and care teams.

You will ensure compliance with NYS DOH regulations, maintain HIPAA, supervise nursing staff, and contribute to wellness and discharge planning while delivering compassionate, resident-centered care.

Qualifications

• Must read, write, speak and understand English.
• Must adhere to all facility policies and procedures.

Responsibilities

• Evaluate resident needs and program capability (initially and quarterly).
• Orient new residents to daily routines and participate in care planning meetings.
• Identify resident adjustment issues and assist resolution.
• Maintain family and community connections for residents.
• Encourage resident participation in community activities.
• Arrange services and entitlements from public/private sources for income, health, mental health, social services, and therapy.
• Assist residents in obtaining a primary physician for health/mental health needs.
• Coordinate work of other case management and service providers (Hospice, CHHA’s).
• Assist residents with discharge or transfer plans.
• Assist in shopping for personal items for residents.
• Oversee and document case management needs (plan of care, medication assistance, fall management).
• Operate within NYS DOH regulations and Resident Rights, HIPAA compliance.
• Supervise Charge Nurses and aides while maintaining staffing schedules.
• Support Wellness Program and Fall Prevention under direction of Wellness Director/Director of Resident Services.
• Conduct meetings with families to foster communication, exemplary resident care, and customer service.
• Attend marketing events as required.
• Maintain on-call schedule and provide consultation to nursing staff.
• Assist with interviewing, hiring, discipline, termination, and supervision of nursing staff.
• Follow Viva Senior Living policies and QA activities.
• Maintain required reporting logs and department budget.
