Job summary

Unicity Care Management is seeking a Senior Living Advisor to guide families through evaluating care needs and selecting appropriate senior living options in New Jersey. You will assess medical, functional, and psychosocial needs and develop tailored care plans prioritizing safety, quality of life, and long‑term stability.

You will educate families on care options, coordinate placements, and collaborate with clinicians to ensure alignment with assessments.

Qualifications

• 3–5 years of experience working with older adults in senior living, care management, hospital discharge planning, home care, or related geriatric services.
• Strong knowledge of local senior living options and in‑home resources in northern and central New Jersey.
• Ability to perform comprehensive assessments and communicate complex information clearly to families under stress.
• Excellent interpersonal skills to build trust, manage sensitive family dynamics, and maintain professional boundaries.
• High level organization and follow‑through with multiple cases, timelines, and stakeholders.
• Proficiency with electronic documentation and Microsoft 365; experience with care management or CRM platforms is a plus.
• Valid driver’s license and ability to travel within the service area.

Responsibilities

• Conduct comprehensive assessments of medical, functional, cognitive, psychosocial, and environmental needs, including safety and support systems.
• Develop individualized care and senior living plans with clear rationales and priorities.
• Educate clients and families on continuum of senior care options, levels of care, and typical costs.
• Coordinate and facilitate senior living placement, tours, applications, and move‑in planning.
• Collaborate with nurses, social workers, and external providers to align recommendations with clinical assessments.
• Serve as primary point of contact and advocate, maintaining regular communication and resolving issues.
