Job summary

Trinity Senior Living Communities in South Bend, IN is seeking a dedicated Community Life Aide for a part-time Day shift (11:00am–7:00pm, includes every other weekend). You will plan, implement and evaluate recreational, social, intellectual, emotional and spiritual activities aligned with each resident's care plan.

You will interview residents and families to set meaningful goals, coordinate with care teams, and document progress in Care Tracker.

Qualifications

• High School Diploma or equivalent.
• Chauffeur license desirable and may be required to obtain a class C driver's license.
• Experience in long-term care or with geriatric populations preferred.
• Superior customer service skills and professionalism.
• Outstanding verbal and written communication skills.
• Ability to adapt to ongoing change and work in a fast-paced, customer-driven environment.
• Interpersonal and relationship-building skills for cross-functional work with residents.
• Self-starter with high productivity and accountability.
• Ability to plan, organize, develop, implement and interpret programs and policies.

Responsibilities

• Assist in planning, implementing and evaluating community life programs.
• Interview residents or families to plan meaningful objectives and goals.
• Serve as on-site community representative to involve residents and families.
• Develop written care plans identifying strengths, needs and goals.
• Observe resident attendance, mood and involvement for progress tracking.
• Promote socialization and physical activity, expand interests and lifelong learning.
• Coordinate with other departments to ensure uninterrupted programs.
• Assist with transportation of residents in a company vehicle.
• Document accurately in Care Tracker per policy and standards.
• Attend scheduled trainings and mandatory in-service sessions.
