Job summary

UPMC Community Care Behavioral Health seeks a full-time IBHS Licensed Professional Care Manager to join the Care Management team in Delaware County. The role is hybrid, with travel in Delaware and surrounding counties, onsite in Media, PA, and some remote work.

Mileage is reimbursed at IRS rates. The Care Manager coordinates care for members at risk, conducts reviews, and provides service authorization and discharge planning.

Qualifications

• Licensed behavioral health professional.
• Experience in case management and care coordination.
• Strong communication and collaboration skills.

Responsibilities

• Makes authorization determinations for medically necessary services.
• Coordinates care with primary care and other providers integral to the member's life.
• Develops outreach plans for assigned members who do not maintain regular contact.
• Monitors and evaluates treatment plans to ensure clinically appropriate services.
• Assists with coordination of information and presentations to providers and agencies.
• Participates in professional development activities and CQI initiatives.
• Maintains client caseload across the continuum and shares information with stakeholders.
