Job summary

UPMC Community Care Behavioral Health seeks a full-time Mobile Professional Care Manager to support the Care Management Department across Mercer and Venango Counties. The role involves flexible hours and travel to meet members in the community, with remaining work conducted remotely.

You will perform assessments that cover behavioral, clinical, social, and environmental needs, coordinate programs and services, and ensure timely documentation.

Qualifications

• Experience coordinating care for behavioral health populations.
• Ability to conduct in-person assessments in community and home settings.
• Strong communication with physicians, clinicians, and providers.

Responsibilities

• Conduct face-to-face member assessments in community, home, or facility settings.
• Collaborate with providers to plan and coordinate treatment and aftercare.
• Identify behavioral, clinical, social, and environmental needs during assessments.
• Link members and families to primary care, behavioral health, and social services.
• Ensure documentation and case management within departmental standards and timeframes.
• Make referrals and provide expertise on community and governmental resources.
• Coordinate care across the continuum with care management, physicians, pharmacy, and other teams.
