Job summary

MemorialCare Health System in Long Beach is seeking a Social Worker, LCSW to provide interventions, advocate for patient self-determination, and supervise staff in the Social Work Services department.

The Licensed Clinical Social Worker will manage a patient caseload, collaborate with care teams, educate families, and coordinate resources to support safe transitions through the continuum of care. This is a full-time, day-shift role on-site at Miller Children’s Hospital.

Qualifications

• Three to five years direct social work experience in an acute healthcare setting preferred.
• Ability to act as a Field Supervisor and/or Preceptor to MSW Student Interns preferred.
• Competency is defined and measured via the Social Worker Job Specific Competency Assessment and Checklist.

Responsibilities

• Independently manages a specific caseload and develops treatment plans.
• Provides leadership and coordination to address protective service issues and advance directives.
• Collaborates with case managers to educate and refer patients and families.
• Supervises staff and provides weekly supervision to MSW staff and interns.
• Educates patients and families to ensure resources are in place to prevent readmission.

Education

Masters of Social Work (MSW) Degree from an Accredited School of Social Work
