Job summary

The City of Hartford’s Health and Human Services Department is seeking a part-time interviewer to support the Chronic Disease Self-Management Program for adults 65+. Under supervision, you will interview applicants to collect identifying information and determine eligibility status in various settings including homes, senior centers, and community offices.

You will connect eligible seniors with health and community resources and maintain records for reporting.

Qualifications

• Graduation from a standard high school, supplemented by college courses.
• One (1) year experience as an Interviewer, or three (3) years in public contact in social services.

Responsibilities

• Interview applicants to obtain identifying information and determine eligibility.
• Interview applicants in homes, community organizations, senior centers, offices, and other locations.
• Determine eligibility and connect older adults with health and community resources.
• Maintain statistical records for monthly, quarterly and annual reports.
