Job summary

The Patient Record Abstractor at UCSF fulfills a role as a Medical Coder, reviewing patient records, discharge summaries, and operative reports to assign standardized codes for diagnoses, procedures, and services.

You will apply CPT, ICD-10-CM, and HCPCS classifications to ensure records reflect care delivered, collaborating with clinicians and billing staff to maintain accuracy and compliance. This role supports audits and continuous data quality improvements.

Qualifications

• 5+ years of professional fee/revenue cycle coding experience
• Hands-on professional coding experience in Interventional Radiology.
• Advanced knowledge of CPT, ICD-10 coding conventions, and clinical documentation requirements.
• Knowledge of federal, state, and commercial payer coding and billing requirements.
• Ability to interpret complex coding and clinical finance information and communicate across clinical and business teams.
• Ability to successfully complete UCSF computer systems and coding/billing application training.

Responsibilities

• Work in complex work queues daily as defined by UCSF Leadership.
• Maintain or exceed a 95% accuracy rate.
• Code complex procedures/accounts requiring advanced expertise in charge capture, workflow, hospital operations, authorizations, and revenue cycle.
• Audit data input to support revenue cycle management.
• Lead on applicable billing, coding, and revenue cycle regulations and communicate these to faculty, management and staff.
