Placements24 is seeking an Elderly Care Coordinator to manage in-home care for seniors, coordinating services, appointments, and family communications. The role focuses on dignity, safety, and personalized support within the Welkom community.

You will lead a team of caregivers, assess client needs, and develop individualized care plans while ensuring adherence to care standards and regulations. Requires 5+ years in geriatric care and a valid driver's license.

Qualifications

Background in nursing, social work, or a related healthcare field.
Minimum of 5 years in geriatric care, home health, or similar role.
Knowledge of common health issues affecting seniors and community resources.
Excellent interpersonal, communication, and problem-solving skills.
Valid driver’s license and reliable transportation for client visits.

Responsibilities

Assess the needs of elderly clients and develop comprehensive, individualized care plans.
Coordinate the scheduling and delivery of home-based care services.
Arrange and escort clients to medical appointments and therapy sessions.
Monitor client health and well-being, reporting changes to healthcare providers.
Act as a liaison between clients, families, caregivers, and medical professionals.
Ensure compliance with all relevant care standards and regulations.
