Job summary

Open Arms Solutions in the Chicago area is seeking a Community Care Coordinator to ensure exceptional in-home care and client satisfaction. You will manage client plans, coordinate with families and caregivers, and oversee care delivery with a focus on personalized service.

Ideal candidates have 3–5 years in high-touch roles within senior care or hospitality, CNA preferred, and proficiency with WellSky and Microsoft Office 365. Travel up to 50% and lifting up to 50 lbs are expected.

Qualifications

• 3-5 years in a high-touch, client-facing role in senior care or hospitality.
• CNA certification strongly preferred.
• Care management experience and caregiver team support.
• Strong communication, interpersonal, and problem-solving skills.
• Knowledge of ADLs/IADLs for senior care clients.
• Experience in private duty/home care service.
• Excellent organizational skills and attention to detail.
• Proactive, service-oriented mindset with passion for extraordinary care.
• Proficiency in WellSky and Microsoft Office 365.
• Valid driver’s license and ability to lift up to 50 pounds.
• Travel up to 50%.

Responsibilities

• Conduct in-depth consultations and assessments to tailor care plans.
• Guide intake process, agency registration, care planning, budgeting.
• Manage a client caseload as main point of contact.
• Provide emergency coverage for client shifts as needed.
• Support hiring, training, and mentoring caregivers.
• Match clients with caregivers based on skills and preferences.
• Perform regular quality assurance visits and check-ins.
• Anticipate evolving client needs and offer proactive solutions.
• Coordinate between clients, families, service teams, and scheduling.
• Maintain confidential records for clients and caregivers.
• Participate in process improvement and system implementations.
