Job summary

Heartland Hospice is seeking a Senior Hospice RN Patient Care Manager to oversee patient care operations, supervise nursing and clinical staff, and ensure adherence to Medicare, Medicaid, JCAHO and ACHC standards.

Reporting to the Executive Director or Administrator, you will coordinate IDG meetings, manage scheduling and visits, handle referrals and insurance verifications, and act as a liaison among clinical teams and external providers.

Qualifications

• Current RN license in state of employment.
• Graduate of an accredited nursing program (ASN accepted with experience).
• Minimum 3 years direct patient care experience.
• Knowledge of hospice and regulatory compliance.
• Strong communication and organizational skills.
• Proficiency with EMR and healthcare software.
• Valid driver’s license and auto insurance.
• Ability to provide skilled nursing care including on-call.

Responsibilities

• Lead hospice patient care operations to ensure quality and compliance.
• Supervise, teach, evaluate, and develop nursing and clinical staff.
• Manage referrals, DME pickups, Medicare eligibility, insurance verification, and patient benefits.
• Coordinate Interdisciplinary Group (IDG) meetings to review care plans.
• Oversee scheduling, visit assignments, and timely patient visits.
• Respond to referrals and clinical inquiries with compassion.
• Serve as liaison between clinical teams, leadership, and providers.
• Ensure adherence to hospice regulations (Medicare/Medicaid/JCAHO/ACHC).
