$10,000 Sign on Bonus!
We are seeking a RN Patient Care Manager Senior to join our team. Reporting directly to the Executive Director or Administrator, you will assist in maintaining a high-performing hospice program by managing patient care activities, supervising staff, and ensuring compliance with all regulations.

• Facilitate and serve on the Interdisciplinary Group (IDG), including presiding over meetings, reviewing patient charts, and coordinating care plans.
• Oversee clinical and administrative functions such as documentation, referrals, insurance verifications, and patient benefit management.
• Supervise clinical staff, assign visits, and ensure timely completion of patient visits.

About You

• Graduate of an accredited nursing school; bachelor’s degree preferred
• Current RN license in your state of residence
• Minimum three years of direct patient care experience and two years in hospice or home health care
• Knowledge of hospice principles, regulations, and best practices (Medicare, Medicaid, JCAHO, ACHC)
• Understanding of managed care and regulatory guidelines
• Ability to manage programs, ensure quality, and maintain compliance
