Senior Hospice Patient Care Manager Leader - RN - $10,000 Sign on Bonus

Heartland Hospice, part of the Gentiva family, is seeking a Senior Hospice Patient Care Manager Leader - RN to join our team. We are industry leaders in hospice, palliative, home health, and personal home care. Our mission is to support patients and families physically, spiritually, and emotionally, ensuring they make the most of every moment.

Position Overview: You will report to the Executive Director or Administrator and assist in maintaining a high-performing hospice program. Your responsibilities include managing patient care activities, supervising staff, ensuring compliance with regulations, and fostering team development.

  • Facilitate and participate in the Interdisciplinary Group (IDG) meetings, overseeing patient reviews, care plans, and team input.
  • Oversee clinical and administrative functions, including documentation, referrals, insurance verifications, and regulatory compliance.
  • Manage clinical staff schedules, assign visits, and ensure timely completion of patient visits.

Qualifications:

  • Graduate of an accredited nursing program; Bachelor’s degree preferred.
  • Current RN licensure in the state of residence.
  • Minimum three years of direct patient care experience, with at least two years in hospice or home health care.
  • Knowledge of hospice principles, industry regulations, and terminally ill patient care.
  • Ability to manage programs, ensure quality, and maintain compliance.
