Quad Recruitment seeks a licensed Pharmacist to join a home infusion team, handling the full clinical picture from therapy assessment to dispensing, monitoring, and ongoing care coordination in the alternate care setting.

The role requires an active state license, 2+ years in acute care (or 1+ year in home infusion), and a strong IV therapy background. You will ensure compliance with USP <797> and regulatory standards while supporting the Pharmacy Manager/Director and care teams.

Formación

• Active pharmacist license in the state of employment.
• 2+ years in an acute-care setting or 1+ year in home infusion therapy.
• Strong IV admixture and IV therapy background.

Responsabilidades

• Assess patient suitability for home care and therapy appropriateness.
• Build and maintain patient profiles and care plans.
• Review labs and perform pharmacokinetic dosing with recommendations.
• Process prescription orders with correct concentrations and storage.
• Dispense medications and program infusion pumps; manage claims and authorizations.
• Educate patients and caregivers on therapy and infusion methods.
• Document care across the full course of therapy, including QA/QC records.
• Ensure USP <797> and state/federal standards for compounding and clean room.
• Oversee staff and train interns; participate in on-call rotations.
