Responsibilities include developing care plans, linking clients to community resources, reviewing progress, and ensuring proper documentation and policy adherence. This role emphasizes compassionate care and outcomes in home-based settings.

Qualifications

• Current state license as required by state law and regulation.
• Minimum of one year medical social work experience.
• Master’s degree in Social Work.

Responsibilities

• Assist clients in understanding social and emotional factors influencing health.
• Develop and coordinate plans of care with clinical input.
• Link clients to community medical and social resources.
• Review progress and adjust services to maximize rehab potential.
• Maintain accurate client data and required documentation.
• Educate clients and caregivers to support optimal functioning.
