Signature Healthcare at Home is seeking a patient-centered RN Clinical Manager to lead our nursing team in the Portland area. You will oversee clinical nursing operations, ensure high-quality, patient-centered care, and drive performance improvements in collaboration with the Director of Clinical Services.

The role includes staff development, regulatory compliance, and coordinating care plans with physicians and the interdisciplinary team.

Qualifications

• Must be an RN with current licensure in the state (preferred).
• Minimum of two years of management experience in home health or related healthcare.
• Demonstrated ability to supervise and direct clinical nursing personnel.
• Strong leadership, communication, and organizational skills.
• Knowledge of state and federal regulations related to home health and nursing practice.
• Must be a licensed driver with an insured automobile in good order.

Responsabilités

• Provides direct oversight of all nursing personnel and nursing-related clinical services.
• Coordinates and oversees patient services provided by nursing staff.
• Guides nursing staff to improve clinical care delivery and performance.
• Assists in managing clinical nursing teams and care planning activities.
• Develops and updates plans of care with appropriate nursing involvement.
• Communicates with physicians and interdisciplinary team members about patient needs.
• Ensures safe, evidence-based nursing practices and regulatory compliance.
• Performs staffing, budgeting, and performance evaluations in collaboration with leadership.
• Supports orientation and ongoing education for nursing staff.
