Qualifications

• Must be licensed as Physical Therapist Assistant in Florida.
• Criminal background check is required.
• Current CPR certification is required.

Responsibilities

• Provide intermittent skilled PT services to patients in their homes under the supervision of the PT.
• Assist in evaluation and development of the rehabilitative plan of care and periodic re-evaluation.
• Prepare clinical and daily progress notes within 48 hours; submit and file as required.
• Perform routine strengthening and therapeutic exercises; assist patients with self-help devices.
• Instruct patient and family in exercises; support teaching of exercise programs.
• Observe and record patient responses and report changes to the PT and physician.
• Attend staff meetings, case conferences, and discharge activities with the team.
