Job summary

BoldAge PACE is seeking a Part-Time Physical Therapist in North Charleston, SC to join the interdisciplinary care team. You will assess participants, plan and provide PT services, and educate families to support community-based participation.

This role emphasizes collaboration with the IDT and delivering high-quality care in the center, clinic, or home settings. Required qualifications include an advanced degree in Physical Therapy, current SC license, and a driver’s license.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Current driver’s license and proof of auto insurance.
• Two years of experience on an interdisciplinary team preferable.

Responsibilities

• Plans, organizes, and implements Physical Therapy services for PACE participants.
• Performs in-person assessments at enrollment and ongoing as determined by the IDT.
• Assesses needs for adaptive equipment and home safety; recommend modifications.
• Plans and administers therapy programs to meet identified goals.
• Documents evaluations and treatment accurately in medical records.
• Educates participants and families in procedures and home programs.
