Responsibilities

• Coordinate and maintain resident and Care Giver schedules to ensure high-quality care delivery
• Create optimal resident and Care Giver matches that support relationships and outcomes
• Monitor scheduling activities and address coverage gaps or changes in resident needs
• Develop contingency staffing plans to ensure uninterrupted service coverage
• Maintain resident and Care Giver information in the software system
• Track and communicate all resident and Care Giver interactions and updates
• Serve as a primary contact for residents, family members, and Care Givers
• Respond to calls professionally and knowledgeably
• Follow up on resident and Care Giver concerns to resolve promptly
• Identify opportunities to enhance services and improve care quality and outcomes
• Manage new resident inquiries and assist onboarding process
