Job summary

Trinity Health PACE is seeking a dedicated Personal Care Assistant to support elderly participants in home-based settings, with occasional facility work. The role includes ADL assistance, safety monitoring, and helping with transportation and activities, with one weekend per month and rotating holidays.

Qualifications

• High School Diploma or equivalent required.
• Minimum 1 year of experience with a frail or elderly population; training from PACE if less than 1 year.
• Experience in long-term care or geriatric programs preferred.
• Must be action-oriented, conflict management, customer focused, adaptable to change, and able to exercise independent judgment.
• Ability to work with minimal supervision and strong written/verbal communication skills.
• Valid driver’s license and use of personal vehicle for home visits.
• Position may require 10% travel within geographic region.

Responsibilities

• Assess and monitor participants and report on services provided and self-care abilities.
• Provide personal care assistance with ADLs, including toileting, showers, eating, and grooming.
• Support physical activity, exercise and walking.
• Identify safety hazards and emergency situations and take corrective action.
• Assist with conversations, recreation activities, meals, snack serving, and housekeeping.
• Report signs of physical, emotional, or behavioral changes to staff or supervisor.
• Escort participants to appointments.
• Assist transportation staff with loading/unloading participants and assist during transport.
