Qualifikationen

• Must have high school diploma or equivalent.
• Experience caring for frail or elderly populations preferred.
• Ability to work with minimal supervision and adapt to changing priorities.
• Excellent written and verbal communication skills.
• Valid driver’s license and access to a personal vehicle for home visits.

Aufgaben

• Engage in ongoing participant assessment and monitoring and report on services provided and self-care abilities.
• Provide personal care assistance with activities of daily living including toileting, showers, eating and grooming.
• Assist in maintaining physical activity, exercise, and walking.
• Identify safety hazards and emergency situations and take corrective action as able.
• Assist with general activities such as conversation, recreation, meals, and housekeeping.
• Escort participants to appointments and assist transportation staff as needed.
