Job summary

Trinity Health PACE seeks a dedicated Personal Care Assistant to provide in-home care and occasional facility support. The role requires one weekend per month and rotating holidays, with assignments primarily at clients' homes.

You will assist with activities of daily living, monitor health status, transport to appointments, and report changes to the center staff. A high school diploma, valid driver's license, and comfort with travel within the region are required.

Qualifications

• High school diploma or equivalent required.
• Minimum of one year experience with frail or elderly population; if less than one year, training provided by PACE upon hiring.
• Experience in long‑term care or geriatric programs preferred.
• Must be able to work with minimal supervision and adapt to change.
• Excellent written and verbal communication and presentation skills.
• Valid driver’s license and use of personal vehicle for home visits.
• Position may require 10% travel within geographic region and use of personal vehicle.

Responsibilities

• Assess ongoing participant status and monitor services and self-care abilities.
• Provide personal care assistance with ADLs (toileting, showers, eating, grooming).
• Support physical activity, exercise, and walking.
• Identify safety hazards and initiate corrective action as able.
• Assist with conversations, recreation, meals, snacks, and housekeeping.
• Identify signs of health or behavioral changes and report to staff.
• Escort participants to appointments as needed.
• Assist transportation staff with loading/unloading and transport in center vehicles.
