Lutheran Child and Family Services of Indiana/Kentucky seeks a Home Based Coordinator to provide clinical leadership, supervision, and operational oversight of our Home-Based Services program in Indianapolis. This role blends direct client care with program management and staff development.

Formación

• Master's degree in mental health-related field.
• Current, valid Indiana clinical license: LCSW, LMHC, or LMFT.
• LCAC (Licensed Clinical Addiction Counselor).
• Minimum of three years of clinical experience in mental or behavioral health.
• At least one year of administrative or leadership experience.
• Strong clinical assessment, treatment planning, and supervisory skills.
• Excellent communication, organizational, and problem-solving abilities.
• Knowledge of trauma-informed, strengths-based, and culturally responsive practices.
• Valid Indiana driver's license and ability to meet LCFS driving requirements.

Responsabilidades

• Provide regular clinical supervision, case consultations, and ongoing professional development for assigned clinical staff.
• Mentor and support staff while promoting accountability, teamwork, and high-quality service delivery.
• Ensure staff complete required training and demonstrate competency in their assigned responsibilities.
• Monitor staff caseloads, service delivery, and client progress to ensure effective and timely interventions.
• Review clinical assessments, individualized treatment plans, and case documentation.
• Conduct timely case reviews and provide clinical guidance to support treatment effectiveness.
• Maintain a small billable caseload providing therapeutic services to individuals and families in home, school, and community settings.
• Provide clinical oversight of crisis intervention, suicide risk assessments, mandated reporting, and incident response.
• Ensure staff follow established safety procedures, reporting requirements, and appropriate escalation protocols.
• Conduct regular case record reviews to ensure documentation is accurate, timely, complete, and compliant with applicable standards.
• Monitor adherence to LCFS policies, licensing requirements, contractual obligations, and accreditation standards.
• Promote ethical practices, confidentiality, professional boundaries, and culturally responsive care.
• Monitor clinical outcomes, service quality, and program performance.
• Identify opportunities to strengthen service delivery and operational effectiveness.
• Participate in PQI activities, program evaluations, and corrective action initiatives.
• Collaborate with leadership and multidisciplinary teams to improve client outcomes and maintain program accountability.
