Job summary

The University of Illinois Chicago in Chicago, IL is seeking a Health Care Utilization Review Coordinator (HURC) to coordinate patient services across the continuum using Relationship-Based Care. You will support care teams, manage utilization and discharge planning, and help referrals across levels of care.

This role requires a nursing degree and an Illinois RN license, with at least five years in Medical Surgical settings and BLS/CPR certification. Not eligible for benefits.

Qualifications

• Graduation from an accredited professional nursing program.
• Current RN licensure with Illinois Department of Financial and Professional Regulation.
• Five years (60 months) of Medical Surgical experience.
• BSN (or higher) required of an accredited nursing program.
• Current BLS/CPR and/or other required unit specific certifications.

Responsibilities

• Advocates for patients, families, and the community.
• Facilitates coordination of patient services in assigned unit.
• Communicates discharge planning process with clinical team.
• Identifies variations in utilization with payors to fund care.
• Documents risk assessment and discharge planning in medical records.
• Collaborates with interdisciplinary team to expedite care.
• Maintains expertise through ongoing education.
