Qualifications

• Graduate of an accredited professional nursing program.
• Current RN licensure in Illinois.
• Five years of Medical–Surgical experience.
• BSN or higher from an accredited program.
• Current BLS/CPR and/or unit-specific certifications.

Responsibilities

• Advocates for patients and families within the care continuum.
• Coordinates discharge planning with the care team to ensure safe, cost-effective care.
• Documents risk assessment and discharge planning per departmental standards.
• Identifies variations in benefit utilization with payors and resources.
• Facilitates interdisciplinary communication to expedite care across the continuum.
• Participates in ongoing education and serves as a care facilitation reference for staff.
• Collaborates with nurses and physicians to teach staff and students.
• Performs other duties and special projects as assigned.
