Avamere Living in Hillsboro, OR is seeking a Director of Health Services (RN) to lead the health services team in an assisted living / memory care setting. You will supervise care delivery, ensure regulatory compliance, and mentor staff to uphold high care standards.

The role requires a nursing degree and an active Oregon RN license, with BSN preferred, plus strong leadership and communication skills for coordinating resident care with families and interdisciplina ry teams.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess an active RN license in Oregon; BSN preferred.
• Experience in assisted living, memory care or geriatric care settings.
• Knowledge of CMS/state regulations and compliant documentation.
• Experience with electronic medical records documentation.
• Strong leadership, communication and training abilities.

Responsibilities

• Resident Assessments: assess residents and update care plans.
• Care Planning & Documentation: develop person-centered plans and document.
• Regulatory Compliance: ensure adherence to state and federal rules.
• Medication Management: oversee medication processes and audits.
• Staff Leadership & Education: mentor care staff and provide training.
• Collaboration & Communication: coordinate with families and IDT.
• Healthcare Coordination: liaise with physicians and hospitals.
• Problem-Solving & Critical Thinking: handle emergencies and changes.
• Quality Improvement: monitor KPIs and drive improvements.
• On Call requirements: participate in after-hours rotations.
