Job summary

Avamere Communities in Seaside, OR, seeks a Director of Health Services (RN) to lead health services in a long-term care setting. The role includes assessing residents, planning care, ensuring regulatory compliance, and guiding the health services team to deliver high-quality, person-centered care.

The DHS will mentor staff, coordinate with doctors and therapists, and participate in audits and quality improvement initiatives to enhance resident outcomes.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, active RN license in the state.
• Experience with CMS and state regulations for ALF/IL/MC settings.
• Experience in electronic medical records documentation.
• Strong leadership, communication and training abilities.
• Knowledge of state and federal regulations and compliance requirements.
• Excellent organizational and time management skills.
• Maintain CPR/BLS during employment.

Responsibilities

• Resident Assessments and ongoing care plan updates.
• Develop and evaluate individualized, person-centered care plans.
• Ensure regulatory compliance and participate in audits.
• Oversee medication management and audits for safety.
• Provide direction and training to health services staff.
• Foster collaboration with residents, families and IDT.
• Coordinate with external providers for continuity of care.
• Apply critical thinking to emergencies and dynamic situations.
• Participate in quality improvement initiatives and KPI reviews.
• On-Call rotation for after-hours, weekends and emergencies.
• Other duties as assigned.
