Job summary

Avamere Skilled Advisors, LLC is seeking a Director of Health Services - RN (DHS) for Avamere at Hillsboro, OR. The role requires leadership of the health services team in an assisted living setting, ensuring regulatory compliance and high quality resident care.

The DHS will lead assessments, care planning, staff training, and coordination with physicians and outside providers, while maintaining documentation and driving quality improvement in a long-term care environment.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active RN license in the state.
• CMS and State Regulations knowledge for ALF/IL/MC facilities.
• Experience with electronic medical records documentation.

Responsibilities

• Conduct resident assessments and update care plans.
• Develop and evaluate individualized, person-centered care plans.
• Ensure compliance with state and federal regulations and policies.
• Oversee medication management and audits for compliance.
• Provide leadership, training and mentorship to health services staff.
• Coordinate with physicians, therapists and hospitals for continuity of care.
• Participate in IDT meetings and care conferences.
• Participate in on-call rotation for after-hours and weekends.
