Job summary

Martin's Point Health Care in Maine is seeking a Care Manager who provides comprehensive, person-centered care management for members and families. You will assess, coordinate, educate, and advocate with clinicians and community services to improve health outcomes and reduce unnecessary hospital utilization.

The ideal candidate has a BSN, at least 5 years of clinical experience, and CCM certification is preferred.

Qualifications

• Bachelor’s degree in nursing required; MSN preferred.
• 5 years of current clinical experience in relevant areas.
• Experience in a managed care, home health or hospital environment.
• CCM certification preferred.

Responsibilities

• Performs comprehensive nursing assessments for new and existing members.
• Applies CMSA, NCQA, TRICARE, and CMS criteria to care management.
• Coordinates care with primary care providers, specialists, and community services.
• Educates and advocates for member-centered goals to improve outcomes and reduce hospital use.
