Job summary

Premier Community HealthCare Group in New Port Richey, FL seeks a Lead Health Coach to help patients build healthier habits through education, outreach, and program development.

You will train Health Coaches, develop engaging curricula for pediatric and adult living classes, and collaborate with a care team to improve health outcomes. The role emphasizes prevention, chronic disease management, and community health.

Qualifications

• Associate degree in public health, health education, nursing, diabetes peer education, or certification as a Community Health Worker.
• Bachelor’s degree preferred.
• At least 1 year of patient education experience required; 3 years preferred.
• Two years with Premier Community HealthCare Group strongly preferred.
• Current BLS certification preferred.
• Ability to obtain Epic certification and Diabetes Education training.

Responsibilities

• Develop pediatric and adult healthy living curricula.
• Facilitate interactive group education sessions virtually and in person.
• Analyze population health data to identify care gaps and support targeted outreach.
• Provide front-line support and guidance to Health Coaches.
• Serve as a subject matter expert for Health Coach policies, procedures, and program improvements.
• Provide individual, telephone, virtual, and group patient education.
• Case manage patients participating in preventative care and chronic disease programs, including diabetes.
• Complete program reporting and collaborate with leadership and community partners.
