Marathon Health is seeking a Certified Health Coach to join the integrated health center team in Port Charlotte, FL. You will provide personalized coaching to help patients manage chronic conditions, prevent disease, and improve overall health.

The role involves one-on-one and group coaching, collaboration with clinicians, and delivering evidence-based programs to support population wellness goals. This hybrid position offers in-person and remote work with a schedule that supports patient access.

Qualifications

• Bachelor’s degree in health science, counseling, psychology, nutrition, social work or related field; NBHWC certification required.
• NBHWC certification required and minimum 2 years’ health coaching experience.
• Experience with one-on-one and group coaching.
• Strong health promotion and chronic condition management background.

Responsabilités

• Conduct individual and group coaching to promote behavior change and wellness; develop personalized wellness plans.
• Collaborate with healthcare providers to offer coaching based on national guidelines.
• Assist patients in scheduling medical appointments and making internal referrals.
• Facilitate health promotion programs such as group challenges and biometric screenings; develop population wellness strategies.
• Participate in client engagement events and coordinate with vendors to inform clients of resources.
• Other duties as assigned.
