Job summary

The Judge Group is seeking an experienced Health Coach for a contract role in Tampa, FL. This position involves health coaching and education for members with chronic conditions, working closely with nurse case managers to assess needs, set goals, and promote positive changes.

The role requires a Bachelor's in a health field, a CDR credential, and active Florida licensure, with at least 3 years of related experience and experience with diabetes, kidney disease, and cardiovascular health.

Qualifications

• Bachelor’s degree in Health Education, Exercise Physiology, Dietetics or related healthcare field.
• CDR credential required.
• Active Florida licensure required.
• Minimum of 3 years of health education, exercise instruction, or patient education experience.
• Experience with diabetes, kidney disease, cardiovascular conditions, or weight-management needs.

Responsibilities

• Conduct behavioral assessments to identify members' knowledge, skills, and behavioral needs.
• Provide individualized health coaching based on objectives and goals.
• Use behavior-change principles to identify barriers and develop strategies to overcome them.
• Coordinate with nurse case managers and provide feedback on progress and concerns.
• Monitor and evaluate coaching interventions and modify approaches accordingly.
• Provide education and support to members managing chronic health conditions.
• Assist members with health goals related to diabetes, kidney disease, cardiovascular health, and weight management.

Job Description

We are seeking an experienced Health Coach to provide health coaching and education to members with chronic health conditions. This position works closely with nurse case managers to assess member needs, establish goals, address barriers, and promote positive behavioral changes.

Responsibilities

• Conduct behavioral assessments to identify members’ knowledge, skills, and behavioral needs.
• Provide individualized health coaching based on identified objectives and goals.
• Utilize behavior modification and behavior-change principles to identify barriers and develop strategies to overcome them.
• Coordinate with nurse case managers and provide feedback regarding member progress, goal attainment, and clinical concerns.
• Monitor and evaluate health coaching interventions and modify approaches as appropriate.
• Provide education and support to members managing chronic health conditions.
• Assist members with health goals related to diabetes, kidney disease, cardiovascular health, and weight management.

Required Qualifications

• Bachelor’s degree in an appropriate healthcare-related field, such as:
  • Health Education
  • Exercise Physiology
  • Respiratory Therapy
  • Dietetics
  • Related healthcare specialty
• Equivalent education and relevant experience may also be considered.
• CDR credential required.
• Active Florida licensure required.
• Minimum of 3 years of related experience in health education, exercise instruction, patient education, or an equivalent combination of education and experience.
• Experience working with patients or members with:
  • Diabetes
  • Kidney disease
  • Cardiovascular conditions
  • Weight-management needs

Preferred Qualifications

• Previous health coaching experience.
• Disease management experience.
• Managed care experience.
• Knowledge of behavioral and/or clinical assessment techniques.
• Knowledge of patient and health education.
• Understanding of behavior-change techniques and principles.
• Understanding of disease-management principles.
