Qualifications

• MD/DO in Internal Medicine, Family Medicine, Geriatrics or similar required.
• Georgia medical license required or eligible.
• Board eligible in Internal Medicine, Family Medicine, Geriatrics or similar.
• DEA number for Schedule II-V required.
• BLS certification from AHA or Red Cross within 90 days.

Responsibilities

• Provide comprehensive care for older adult patients with acute and chronic illnesses.
• Assess, diagnose, treat and manage acute and complex chronic health needs.
• Maintain accountability for patient care and outcomes.
• Build trusting relationships with patients and provide ongoing health coaching.
• Coordinate with hospitalists and transitional-care teams; collaborate with case managers and specialists.
