Job summary

Qualifications

• Education: High School Diploma or GED.
• Experience: Three (3) years as a Family Support Worker.
• Must be 18+, have telephone accessibility, reliable transportation and valid driver’s license.
• Willing to work beyond normal health department hours when necessary.

Responsibilities

• Conduct home visits to establish rapport and service plans.
• Provide regularly scheduled home visits and follow-up.
• Document referrals, progress, and services for program evaluation.
• Engage families to develop parenting skills within the HANDS curriculum.
