Job summary

Monarch Healthcare is seeking an Employment Support Professional III to help individuals achieve competitive employment in their community. You will develop personalized dream plans, identify partner organizations, and update career profiles with input from the individual and care team.

The role emphasizes benefits education, disclosure discussions, and holistic interventions. Travel may be required as part of community-based duties and meetings.

Responsibilities

• Help individuals achieve employment goals through outcome measures, skills training, and personalized dream plans.
• Identify partner organizations for volunteer or job opportunities and maintain relationships.
• Complete and update career profiles using information from individuals, caregivers, and past employers.
• Explain how earnings affect benefits and discuss disclosure of psychiatric status with employers.
• Develop holistic, integrated interventions and identify accommodations for Deaf/Hard of Hearing, LEP, and visually impaired individuals.
• Coordinate services to develop and update individual employment plans and IPS-based job development.
• Perform job development activities tailored to each person and create task analyses.
• Ensure individuals perform job tasks to employer satisfaction and provide follow-along supports.
• Educate employers and provide follow-up contact per agreement with individuals supported.
• Provide outreach to individuals who miss appointments and determine barriers to employment plan.
• Spend 65%+ of scheduled hours in the community executing job duties for caseloads.
• Support career development, salary/advancement requests, and education opportunities.
• Meet placement and billing goals, adapt schedules as needed, and complete required documentation.
• Communicate needs/progress to the team and help with transportation and community resource access.
• Collaborate with behavioral health providers, families, housing, and transportation services.
• Attend trainings, maintain certifications, and comply with policies and regulations.
• Possibly travel and driving may be required.
