Job summary

The Larimer County Department of Human Services is seeking a Community Support Specialist III to assess eligibility for LTC, Adult Financial, TANF, Medicaid, and SNAP. You will gather information, explain program requirements, and determine initial and ongoing eligibility while coordinating referrals to related resources.

This hybrid role requires at least one in-office day per week and reliable internet for remote work.

Qualifications

• Two (2) years' experience in a directly related field including one (1) year of experience equivalent to that of a Community Support Specialist II or in the performance of similar duties and responsibilities in a human services program required.
• High school diploma or GED required.
• Bilingual (English/Spanish) skills preferred.

Responsibilities

• Gather information submitted by applicants and recipients regarding eligibility for LTC, Adult Financial, TANF, Medicaid, SNAP, and other programs.
• Inform applicants of requirements and benefits; help complete forms; secure documents; notify about appeal rights.
• Approve or deny eligibility promptly according to state statutes.
• Enter and update case information in the database.
• Maintain caseload under supervision and case review.
• Review monthly change reports for ongoing eligibility and re-determination.
• Provide referrals to related resources and agencies.
