Job summary

UPMC Mercy is seeking a Casual Discharge Plan Manager to support the Emergency Department. This role involves coordinating post-discharge needs and acting as a liaison among care teams to optimize transitions and resource use.

Ideal candidates have an MSW and LSW/LCSW. RNs and social workers are encouraged to apply for roles in care management, case management, or care coordination within the new Clinical Care Coordination and Discharge Planning team.

Qualifications

• Preferred candidates will have an MSW and LSW/LCSW.
• Roles involve discharge planning and care coordination across care settings.
• Experience in hospital discharge processes is advantageous.

Responsibilities

• Identify clinical, psychosocial, historical, financial, cultural, and spiritual needs guiding discharge planning.
• Complete detailed assessments to understand medical and social factors and discharge needs.
• Facilitate teams to develop safe and efficient discharges and coordinate post-hospital care.
