UPMC Mercy Health Center is hiring a Casual Discharge Plan Manager to support the Emergency Department. This role involves coordinating post-discharge needs, collaborating with the care team, and ensuring timely resource utilization for optimal patient outcomes.

RNs and social workers work together in discharge planning under the new Clinical Care Coordination and Discharge Planning team. Responsibilities include comprehensive assessments, coordinating safe discharges, and serving as a liaison

Qualifications

• Coordinate post-discharge needs with care team to ensure timely and appropriate services.
• Assess clinical, psychosocial, financial, and cultural factors influencing discharge planning.
• Communicate effectively with patients, families, and multidisciplinary teams.

Responsibilities

• Identify patient needs guiding discharge planning with health-literacy considerations.
• Complete comprehensive assessment of medical and social factors for discharge readiness.
• Facilitate safe and efficient discharges with post-hospital care arrangements.
• Communicate discharge plans with patient, caregivers, and care team; adjust plans as needed.
• Act as liaison between hospitals and post-acute care facilities and providers.
• Ensure compliance with policies, ethics, and regulatory requirements.
• Contribute to department operations and staff mentorship.
• Engage in process improvement initiatives.
