Qualifications

• Nurse track requires BSN; 10 years experience may substitute.
• Non-nurse track requires MSW or master's in health and human services; 10 years can substitute.
• RN license required; LSW/LCSW or education-appropriate license required.
• CCM/ACM or other nursing or social work certification preferred.
• BLS/CPR required; Act 33/34/73 Clearances required.

Responsibilities

• Coordinate discharge planning from admission to post-discharge.
• Advocate for patient/family access to services and protection of health and safety.
• Identify clinical, psychosocial, financial, cultural, and spiritual needs to guide planning.
• Complete detailed patient assessments to determine self-care capacity and post-hospital needs.
• Collaborate with a multidisciplinary team to develop an individualized, safe care plan.
• Coordinate transition plans integrating patient goals with team assessments and resources.
• Serve as liaison between patients and care team and manage discharge milestones.
• Maintain knowledge of local resources and post-hospital care partners.
