Job summary

UPMC Presbyterian Shadyside seeks a Discharge Plan Manager to support Clinical Care Coordination and Discharge Planning. The role focuses on safe, smooth transitions from inpatient care to home or related settings, coordinating with patients, families, and care teams to meet medical, social, and logistical needs.

Responsibilities include assessing discharge readiness, advocating for patient access, coordinating services across settings, and ensuring timely post-hospital arrangements.

Qualifications

• RN Diploma or Associate's degree is required.
• Bachelor's degree in social work or a related health/human services field is required; a Master's degree preferred.
• At least one year of discharge planning/care coordination experience is required.

Responsibilities

• Work with patients from admission to post-discharge to ensure readiness for discharge.
• Advocate for patients, families, and caregivers to access needed services and protect health and safety.
• Identify clinical, psychosocial, financial, and cultural needs to guide discharge planning.
• Complete detailed assessments to determine self-care capacity and need for post-hospital services.
• Coordinate with a multidisciplinary team to develop individualized, safe discharge plans.
• Incorporate patient goals and team input to finalize a transition plan.

Education

RN Diploma or Associate's degree
