Job summary

Connections Health Solutions in Kirkland, WA, seeks a Case Manager III to facilitate admissions, treatment planning, discharge planning, and crisis management. Qualifications

• Master's degree in behavioral health or related field required.
• Active license as LMHP/LICSW/LMHC/LMFT or eligible associate license.
• Minimum 3 years of mental health or related experience.

Responsibilities

• Facilitates admission intakes and treatment/discharge planning.
• Coordinate care with interdisciplinary treatment team and external providers.
