Job summary

Trinity Health in Des Moines, IA is seeking an RN Case Manager to support MercyOne Care Coordination with night shift responsibilities. You will partner with physicians, insurers, and care teams to ensure patients receive appropriate care across the continuum.

You will review admission status, coordinate services, assist with discharge planning, and connect patients to resources, driving patient-centered care and safe transitions.

Responsibilities

• Drive Patient-Centered Care Coordination across the care continuum.
• Coordinate resources and services to support patients throughout their healthcare journey.
• Collaborate with physicians, social workers, therapists, wound care specialists, and other team members.
• Assist with discharge planning, transitions of care, and community resource connections.
• Help patients access primary care providers, medical services, and support programs.
