Job summary

McGregor PACE in Ohio seeks a Community Resource Liaison to connect participants in the PACE program with available community resources, helping them stay healthy and independent in their homes and communities. Qualifications

• Experience in community outreach or social services preferred.
• Strong interpersonal and written communication skills required.
• Knowledge of local community resources and services beneficial.
• Ability to work independently and as part of a team.
• Comfort with basic computer skills and MS Office applications.

Responsibilities

• Coordinate and follow up with community agencies and service partners.
• Support participants with housing, social support and other needed services.
• Identify barriers to care and communicate them to the MSW/LSW.
• Assist with scheduling and coordinating community-based services as directed.
• Maintain accurate and confidential participant records and documentation.
• Perform other duties as assigned by the MSW/LSW.
