Job summary

McGregor PACE is seeking a Community Resource Liaison to connect participants with available community resources, helping them stay healthy and independent in the community. You will provide information about accessing services and work with an interdisciplinary team to coordinate supports.

Reporting to the social worker, you will be part of a collaborative team, coordinating with housing services, social supports, and other service partners to reduce barriers to care and ensure timely,

Qualifications

• Experience in community outreach or social services is desirable.
• Strong interpersonal communication and writing skills.
• Knowledge of local community resources and services.
• Ability to work independently and as part of a team.
• Computer skills and knowledge of Microsoft Office Suite.

Responsibilities

• Coordinate and follow up with community agencies, healthcare providers, social service organizations and other service partners.
• Support participants with housing, social support, and other needed services.
• Help identify barriers to care and communication concerns to the MSW/LSW.
• Assist with scheduling and coordinating community-based services as directed.
• Maintain accurate, timely and confidential participant records and documentation.
• Perform other duties as assigned by the MSW/LSW.
